All custom orders must be placed by these dates to guarantee 2023 benefits. Please note a quote of benefits and/or authorization does not guarantee payment.  PLEASE NOTE THAT WE DO NOT ACCEPT UHC OR AETNA, we ask that you please update your records before sending benefit verifications for these insurances as we are not contracted.  Payment of benefits are subject to all terms, conditions, limitations and exclusions of the member’s contract at time of service.
